Our office is in the Life Center Building next to the Alta
View Hospital in Sandy, Utah. It is easy to find by
taking either 9400 South or 10600 South from I-15 to
1300 East.
We are located on the bottom floor. On the west side of
the building there are 4 temporary handicap parking
slots. These are for our patients. Our door is located
on the north west corner of the building.
